Greater Portland Health is planning to offer primary care services to Portland Public Library patrons who are seeking a primary care provider (PCP). As a federally qualified health center, Greater Portland Health provides care to all individuals regardless of insurance status or ability to pay. The team will consist of a nurse or nurse practitioner and a Community Health Worker, who will be available to provide services such as wound care, medication management, and assistance with connecting patients to ongoing primary care and community resources. This event will take place on the first Wednesday of every month.
